A useful handover prevents this: it preserves the family’s preparation and gives your team a specific purpose for the conversation.

Give the next colleague a clear task

Write the handover from the perspective of the person preparing the intake conversation. They should see what they can build on and which organisational answer is still missing:

  • Already explained: which parts of the process does the family know?
  • Participants: with whom was the next conversation discussed?
  • Outstanding task: who confirms the appointment, format or response?
  • Purpose: what does the family want to understand during this exchange?

A useful entry might read: “Daughter and father want a joint conversation; introductory meeting still to be explained; appointment only proposed so far; Ms Neumann checks feasibility and replies to the daughter.” This prevents a proposed appointment from accidentally becoming a confirmed one.

Ask the receiving colleague to acknowledge the task briefly. If they need information before the conversation, they should know whether it has already been requested. The family should not have to assemble documents or answers again simply because responsibility has changed internally.

Continue organisational questions purposefully

Start the next conversation with a short recap: “You would like to discuss together how an introductory meeting works. My colleague recorded your preferred start.” Allow corrections. The father may now wish to speak by himself first, or the requested timing may have changed.

If the family raises new specialist questions, identify who will address them. A short handover note does not require a precautionary collection of additional health information. What matters is that the specific question reaches the right person and the family understands what response will follow.

If something remains open after the conversation, record a task rather than a broad status such as “being processed”. “Check appointment options and reply” is actionable. “Intake prepared” does not show whether a response is still owed by the family or your service.

Can the office already give a start date?

Distinguish the requested start, a conversation appointment and an actually agreed care arrangement. An organisational handover or intake conversation does not confirm when visits will start.

What if the family changes information?

Record the change with a date and inform whoever prepares the next conversation. Revisit only the questions affected, rather than repeating the entire first contact.
